Between January 2020 and December 2021, life expectancy declined in the U.S. and in other countries.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#2\" target=\"_blank\" rel=\"noopener\">2<\/a><\/sup> Since the pandemic remains an ongoing threat to global health and well-being, we updated our international comparison of health systems using 2019 data. We compared U.S. health spending, outcomes, status, and health care utilization with Australia, Canada, France, Germany, Japan, the Netherlands, New Zealand, Norway, South Korea, Sweden, Switzerland, and the United Kingdom. In addition, we compared the performance of the U.S. health system with the average of 38 high-income countries using currently available OECD data. For our analysis, data were taken from the Organisation for Economic Co-operation and Development (OECD) and other international sources (see details in the<a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#methods\" target=\"_blank\" rel=\"noopener\">Bu \u00c7al\u0131\u015fmay\u0131 Nas\u0131l Y\u00fcr\u00fctt\u00fck<\/a>\u201cHow We Conducted This Study\u201d section). We used the most recent data available for each metric we examined. This means that results for certain countries may reflect the peak period of the COVID-19 pandemic; during this period, mental health conditions increased, essential health services were disrupted, and patients may not have received care at the same level.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#3\" target=\"_blank\" rel=\"noopener\">3<\/a><\/sup>\n                <\/div>\n            <\/div>\n            <div class=\"article-body article-body--boxed article-body--navy-blue\">\n                <div class=\"article-body__content\">\n                    <h3>Key Findings<\/h3>\n                    <ul>\n                        <li>Health spending, both per capita and as a share of GDP, is much higher in the U.S. than in other high-income countries. However, the U.S. is the only country without universal health coverage.<\/li>\n                        <li>The U.S. has the lowest life expectancy at birth, the highest death rates for preventable or treatable conditions, some of the highest maternal and infant death rates, and some of the highest suicide rates.<\/li>\n                        <li>The U.S. has the highest rate of people living with multiple chronic conditions and an obesity rate nearly twice the OECD average.<\/li>\n                        <li>Americans see doctors less often than people in most other countries and have among the lowest rates of practicing physicians and hospital beds per 1,000 people.<\/li>\n                        <li>While the U.S. has among the highest rates of breast and colon cancer screening and influenza vaccination, its COVID-19 vaccination rate lags behind many countries.<\/li>\n                    <\/ul>\n                <\/div>\n            <\/div>\n        <\/div>\n countries, health spending has risen faster than economic growth since the 1980s.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#4\" target=\"_blank\" rel=\"noopener\">4<\/a><\/sup> This increase has been driven by medical technologies, rising prices in the health sector, and greater demand for services.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#5\" target=\"_blank\" rel=\"noopener\">5<\/a><\/sup> When the COVID-19 pandemic began in 2020, health spending rose rapidly in nearly all countries, as governments tried to contain the spread of the disease through COVID testing, vaccine development, relief funds, and other measures.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#6\" target=\"_blank\" rel=\"noopener\">6<\/a><\/sup> Since then, spending growth has slowed, but it still remains higher than in previous years.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#7\" target=\"_blank\" rel=\"noopener\">7<\/a><\/sup>\n\nIn 2021, the U.S. devoted 17.8% of its gross domestic product (GDP) to health spending, nearly twice the average of OECD countries.\n\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<div class=\"article-body article-body-- spending per person in the U.S. is nearly twice that of Germany, the closest country, and four times that of South Korea. In addition to public insurance, some countries also offer the option to purchase private insurance. In France, nearly the entire population has both private and public insurance. For example, life expectancy at birth in the U.S. was 77 years in 2020 \u2014 three years below the OECD average. Temporary data show that life expectancy in the U.S. declined further in 2021.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#9\" target=\"_blank\" rel=\"noopener\">9<\/a><\/sup>\n\nLife expectancy in the U.S. also masks racial and ethnic inequalities.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#10\" target=\"_blank\" rel=\"noopener\">10<\/a><\/sup> In 2019, the life expectancy of non-Hispanic Black Americans (74.8 years) and non-Hispanic American Indian or Alaska Native people (71.8 years) was four and seven years lower, respectively, than that of non-Hispanic White people (78.8 years). Hispanic Americans\u2019 life expectancy (81.9 years) is higher than that of White Americans and is similar to life expectancy in the Netherlands, New Zealand, and Canada. Temporary data show that life expectancy in the U.S. declined further in 2021.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#9\" target=\"_blank\" rel=\"noopener\">9<\/a><\/sup>\n\nLife expectancy in the U.S. also masks racial and ethnic inequalities.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#10\" target=\"_blank\" rel=\"noopener\">10<\/a><\/sup> In 2019, the life expectancy of non-Hispanic Black Americans (74.8 years) and non-Hispanic American Indian or Alaska Native people (71.8 years) was four and seven years lower, respectively, than that of non-Hispanic White people (78.8 years). Hispanic Americans\u2019 life expectancy (81.9 years) is higher than that of White Americans and is similar to life expectancy in the Netherlands, New Zealand, and Canada. In Norway, by comparison, there were 1.6 deaths per 1,000 live births. Women in the U.S. have long had the highest maternal mortality rate due to pregnancy and childbirth complications. In 2020, there were nearly 24 maternal deaths per 100,000 live births in the U.S., a rate more than three times higher than in most other high-income countries we studied. The data were released in December 2022. Although data collected by the OECD reflect the highest standards for international comparison, one limitation is that differences in countries\u2019 methods of collecting health data may be obscured. Full details on how indicators are defined and country-level definitional differences can be obtained from the OECD.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#24\" target=\"_blank\" rel=\"noopener\">24<\/a><\/sup>\n<h4>2020 Commonwealth Fund International Health Policy Survey<\/h4>\nFor the 2020 Commonwealth Fund International Health Policy Survey, nationally representative samples of adults age 18 and older were collected from Australia, Canada, France, Germany, the Netherlands, New Zealand, Norway, Sweden, Switzerland, the United Kingdom, and the United States. Surveys in each country were conducted using random-digit dialing based on both landline and mobile telephone sampling designs or, where possible, through address-based sampling frames. In Norway, Sweden, and Switzerland, participants were drawn from national population registries. In the U.S., an address-based sampling frame was also used to provide a representative sample. Participants in Sweden, Switzerland, and the U.S. were able to complete the survey by landline, mobile phone, or online. The Commonwealth Fund supported international surveys regularly and, in some countries, supported larger sample sizes to enable equity analysis. Final sample sizes ranged from 607 to 4,530 respondents. The survey research firm SSRS was commissioned to conduct the survey between February and May 2020 in collaboration with country contractors. The field period varied from four to 15 weeks across countries. Response rates ranged from 14% to 49%. Data were weighted by country to account for sampling design and differences in selection probabilities.\n<h4>Our World in Data<\/h4>\nThis analysis used data from Our World in Data, which compiles data from many countries and regions around the world. Data on COVID-19 deaths and COVID-19 vaccination rates were current as of January 2023. One limitation of both datasets is that they report only confirmed deaths and reported vaccination rates; Our World in Data notes that these may underestimate actual totals. Full details about how indicators are defined and country-level data collection differences are available from Our World in Data.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#25\" target=\"_blank\" rel=\"noopener\">25<\/a><\/sup>\n<h3>Limitations of Our Analysis<\/h3>\nThe effects of the COVID-19 pandemic, job loss, and delayed or forgone necessary care likely worsened health outcomes.<sup><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2023\/jan\/us-health-care-global-perspective-2022#26\" target=\"_blank\" rel=\"noopener\">26<\/a><\/sup> Therefore, some country-level analyses should be interpreted cautiously, because data for 2020 and afterward are not available for every country or every metric. As businesses and health facilities reopened and returned to pre-COVID operating levels, the global health burdens experienced during the pandemic may continue. Another limitation of our study is that the data were not broken down by race and ethnicity.
